dont forget the neck in SP heroic last boss, the items you can get with badges of justice, the BoE and BoP stuff made by proffessions and the BoE world drops you can get off AH

Since you're including heroic loot, don't forget the heroic badges rewards:

Choker of Vile Intent - neck
Bloodlust Brooch - trinket
Blood Knight War Cloak - back

Which are far better for the slots than any instance drop in most cases.

Socketed with 3x 8agi gems, Stalker's Helmet of Second Sight is better, dps wise.
