So blizzard posted saying a hotfix has been deployed to fix this bug, yet when my guild tries to finish off the 10 man run tonight, we enter the instance, speak to the guy to start the fight, he disappears, and the floor stays, lovely.
An hour later, after trying soft resets, and well, pretty much anything you could think of, we give up and hit ulduar.
I've done some searches and it seems there isn't an actual fix once the bug happens, so, this is basically a QQ post, what's the deal with this? Why has it not been fixed!?
Had very same problem too. Soft reset didn't work, so we went try Malygos in 6mins. After we were done (in 5 minutes actually ;>), NPC was up again and fortunately this time Lich King appeared normally and smashed the floor.

A fix was put in place to remedy instances which already had suffered this inconvenience, as can be seen here:
It may appear broken at first, so trigger the event, and if it fails to work, then soft reset the instance. Seems to do the trick.
We were going to have some tries on Anub tonite before the server went down and are experiencing the same problem as posted above. Tried the suggestion by the blue on Eu Balnazzar - didn't work. The first time the guy simply wouldn't start the event. After reset the guy wasn't appearing for us at all.
Tried a GM call but the server goes down in about 30 minutes so that will be a no go. Very disappointed.
On a side note; I wasn't keyed to the group who had downed the previous 10 man hc hardmodes. However when I zoned in I became saved to the instance with no usual "you are entering a partially completed instance, click to confirm" message. Is that a bug?
